The most memorable jumpscare in my gaming experience is a giant electrofish in Serious Sam First Blood, sewers level. The first time I played it I was probably around 15 years and fuck, this thing scared me for real.
It's all murky water where you see almost nothing, there's scary sounds, and when you press the switch underwater, this giant mouth appears, full of teeth, and it shoots you with electricity that shakes your character, so you can't aim and kill it. Each time I replayed this level later, I was like "Fuck you, I just shower this place with rockets BEFORE jumping in the water".
